Direct Debit Service Agreement and Request
For licensees wishing to pay a renewal fee or insurance premiums via direct debit using BSA's Phone Pay service. This form must be returned to BSA at least 24 hours before calling through to authorise payment.
NOTE: BSA can not debit money without your authority. You must call 1300 272 272 to complete these transactions and authorise the direct debit payment.
