  • Acknowledgement Termite Protection System
    To provide to home owners to acknowledge discussion of the termite protection system being used
  • Additional / Replacement Card / Certificate Request Form 4A

      • Certificate of Currency for Indemnity
        To be completed by Insurance Broker to confirm PI Insurance Policy held is compliance with Section 9 of the Financial Requirements for Licensing (to see which licence classes are required to hold PI Insurance (click here)
      • Company Licence Application
        To be used by a company when applying for a licence
      • Company Nominee
        Company to complete when adding, changing or removing a nominee supervisor
      • Direct Debit Service Agreement and Request
        For licensees wishing to pay a renewal fee or insurance premiums via direct debit using BSA's Phone Pay service. This form must be returned to BSA at least 24 hours before calling through to authorise payment. NOTE: BSA can not debit money without your authority. You must call 1300 272 272 to complete these transactions and authorise the direct debit payment.

      • Declaration of Estimated Annual Turnover
        Declaration form to be used by licensees who do not have to comply with the FRL and can provide PI Insurance and EAT 
      • Fire Occupational Licence Application

        Applicable for those applying for a Fire Licence
      • Individual Licence Application
        To be used by applicants for an individual licence (trade contractor, builder, nominee supervisor and designer)
      • Insurance Notification
        Payment of premium amount (can not be returned via fax)
        • Monies Owed Complaint Form

        • Mutual Recognition Application

          Form for interstate licencees applying through mutual recognition
        • Nominee Form
          Nominee supervisor to complete when commencing or ceasing as a company nominee
        • Notice in Relation to Changes of Particulars Form 7
          Change of contact details, office holders or name (including trading name)
        • Notification of Offence
